It is in both yours and the
Health Systems Trust’s (HST) best interest to perform integrity
assessments prior to employment.
An integrity assessment involves compiling a comprehensive background check relevant to the
job that will be performed.
One or more of the following methods are used:
•Reference check with referees as supplied
•Qualification verification
•Professional body registration verification (if applicable)
•Credit and/or criminal check
I hereby voluntarily authorise and consent to HST conducting an integrity assessment on me.
I also give consent for HST to contact all previous employers, educational institutions and
referees cited in this application in order to obtain and verify the accuracy of information
contained herein.
I also consent to HST, if necessary, requiring me to undergo a medical examination with a
registered medical practitioner.
I also hereby release HST and its representatives from liability for seeking, gathering and
using such information to make employment decisions, and release all other persons or
organisations from liability for providing such information.
I understand that any misrepresentation or material omission made by me in this application
will be sufficient cause for cancellation of this application or immediate termination of
employment if I am employed, whenever it may be discovered.
I acknowledge that should I be offered employment by HST, a formal contract setting out the
terms and conditions of the employment agreement will be provided for signature by both
parties.
