Description

The Health Systems Trust (HST) seeks to appoint four Community Agents for the Breast Cancer Project in Zululand and City of Johannesburg. To assist with community tracing activities for breast cancer patients who require linkage or re-engagement to treatment. To educate and support breast cancer patients through structured and planned home-visits. To improve treatment adherence by educating and counselling breast cancer patients. This is a fixed-term contract, renewable based on performance and funding.

Key Responsibilities

• Develop a system to monitor patient navigation needs and track referral success rates on a monthly basis according to the project navigation guidelines. • Actively transfer and check the successful transfer of patients to treatment. • Find previously diagnosed patients who require linkage to the next level of care. • Improve treatment outcomes by ensuring adherence to treatment and retention in care. • Develop community tracing plans timeously. • Conduct community education and awareness. • Report daily to the supervisor. • Adhere to HST financial policies. • Effective use of resources. • Management of assets within policy. • Meet targets and report timeously.

Qualification Required

• Certification as a Community Health Care Worker will be an advantage • Fluent in IsiZulu, English and any other spoken South African languages

Experience Required

• Ability to work independently and as part of a multidisciplinary team. • Ability to influence change and build strong relations with various stakeholders. • Knowledge of Microsoft Office applications (Excel, Word, PowerPoint, Outlook). • Ability to impart knowledge and communicate with empathy. • Basic knowledge of breast cancer screening, diagnosis, treatment referral pathways, survivorship support and palliative care referral processes. • Knowledge and application of Batho Pele principles, confidentiality, informed consent and ethical handling of patient information. • Strong communication, interpersonal, organisational and problem-solving skills. • Ability to work with multidisciplinary teams, health facilities, community structures and partners.

Remuneration

A salary package commensurate with the successful candidate’s experience and qualifications and in line with HST’s job grading system will be offered.
