Description

Health Systems Trust (HST) seeks to appoint a Professional Nurse for the Breast Cancer Navigation Project in Gauteng, City of Johannesburg District. To provide professional nursing leadership, clinical oversight and supportive supervision to the breast cancer navigation team, ensuring that clients are supported across the breast cancer continuum from referral through diagnosis, treatment initiation, follow-up, survivorship and palliative care as applicable. To work closely with the Department of Health, facilities, oncology services and community stakeholders to strengthen timely access to breast cancer services, reduce barriers to care, monitor programme performance and support quality improvement in line with applicable policies, guidelines and reporting requirements. This is a fixed-term contract, renewable based on performance and funding.

Key Responsibilities

• Provide clinical oversight and supportive supervision to breast cancer navigators to ensure patient-centered, ethical and evidence-informed navigation services. • Coordinate referral pathways for clients with suspected or confirmed breast cancer, including linkage to diagnostic, surgical, oncology, psychosocial and palliative care services. • Support navigators to identify and address barriers to care, including missed appointments, delayed results, transport challenges, health literacy gaps and psychosocial needs. • Facilitate effective communication between clients, families, healthcare facilities, oncology teams, community partners and the project team. • Ensure confidentiality, informed client engagement and respectful care throughout the navigation process. • Provide technical support, training, and mentorship to DoH facility and community- based staff. • Adhere to HST financial policies • Effective use of resources • Management of assets within policy • Meets targets and reports timeously • Plan, coordinate and monitor daily breast cancer navigation activities across assigned facilities and communities. • Supervise breast cancer navigators, allocate work, review caseloads and provide guidance on complex client cases. • Maintain clear referral, tracking and feedback systems to support timely diagnosis, treatment initiation and continuity of care. • Submit accurate daily, weekly and monthly programme data and narrative reports to the Project Manager and relevant stakeholders. • Participate in project, facility and stakeholder review meetings to discuss performance, bottlenecks and quality improvement actions. • Use data to identify service gaps, strengthen navigation processes and inform corrective action plans. • Perform any other reasonable duties allocated by the Project Manager in line with the role. • Identify training and competency needs of breast cancer navigators and develop supportive supervision plans. • Provide mentoring, coaching and case-based learning to navigators on breast cancer pathways, communication, documentation and client support. • Facilitate orientation and refresher training on project tools, referral processes, confidentiality and quality standards. • Participate in relevant professional development, clinical updates, meetings and learning opportunities. • Contribute to documentation of good practice stories, case studies and lessons learned.

Qualification Required

• A Nursing Diploma or Degree in Nursing is essential. • Current registration with the South African Nursing Council as a Professional Nurse. • Fluent in IsiZulu, English and any other spoken South African languages

Experience Required

• Experience in oncology navigation, breast cancer programme coordination, monitoring and evaluation or quality improvement. • Knowledge and experience of electronic or paper-based patient tracking and data management systems. • Additional training in oncology, palliative care, counselling, quality improvement or project management would be an advantage.

Remuneration

A salary package commensurate with the successful candidate’s experience and qualifications and in line with HST’s job grading system will be offered.
